Permanent non-surgical alarplasty using Dr Hotaki’s method
My approach to reshaping the nasal wings, without surgical skin removal or external stitches.
My approach to reshaping the nasal wings
The shape of the nasal wings influences the proportions of the whole face. In my practice, I use my own permanent non-surgical alarplasty method to change their position and contour while preserving facial individuality. My approach works with the tissues without surgical skin removal.
What distinguishes my method
I use a soft, flexible, non-absorbable implant and my custom needle. An individually planned placement pattern allows me to work with tissue position and create mechanical support. The term permanent refers to the use of a non-absorbable implant.
No skin removal. No external stitches.
The procedure takes 30–45 minutes under local anaesthesia. It does not involve surgical skin excision or external stitches. My approach focuses on minimal tissue trauma and a short recovery period; recovery time depends on the individual tissue response.
Before and after non-surgical alarplasty
These photographs show a result achieved using my method. I plan each correction around the original shape of the nasal wings, asymmetry and overall facial proportions. The patient has consented to publication of these photographs.

Individual proportions guide my planning
I begin by assessing the nose in the context of the whole face. My priority is to determine which change in nasal-wing shape suits the individual patient. I therefore plan each correction individually and assess the right and left sides separately.
