By submitting the request form on the website https://drhotaki.com (the "Site"), I, acting freely, of my own will and in my own interest, and confirming my legal capacity, give my consent to Individual Entrepreneur Khotaki Nur Rahman (OGRN 1027700067328, INN 773612225602, registered address: Derbenevskaya Naberezhnaya 11, "Pollars" Business Center, Moscow, Russian Federation) (the "Operator") to process my personal data provided by me when filling in the request form on the Site, on the terms set out below.

This consent is given in accordance with Federal Law No. 152-FZ "On Personal Data" dated 27 July 2006 and supplements the Operator's Privacy Policy regarding the processing of personal data, published on the Site at https://drhotaki.com/privacy (the "Policy"), with the content of which I have familiarized myself.

1. LIST OF PERSONAL DATA COVERED BY THIS CONSENT

This consent covers the processing of the following personal data:
— first name and last name;
— city of practice;
— phone number / WhatsApp number;
— e-mail address;
— specialization and accreditation details;
— area of training I am interested in;
— information on whether I have my own client base (without disclosing any personal data of those clients);
— any other information I provide in free form in the "Comment" field.

I confirm that the data I provide is accurate and up to date, does not belong to any special category of personal data (health data, biometric data, or other special categories), and does not constitute personal data of any third party.


2. PURPOSE OF PROCESSING

The Operator processes my personal data solely for the purposes of:
— contacting me regarding the request submitted through the form on the Site, including advising me on training matters and the area of training I indicated;
— clarifying the details of my request and providing the information I requested.

The Operator does not use my personal data for any purpose unrelated to processing the submitted request, and does not carry out any automated processing of personal data that produces legal effects concerning me.


3. LIST OF OPERATIONS WITH PERSONAL DATA

This consent covers the Operator's performance of the following operations with personal data: collection, recording, systematization, accumulation, storage, clarification (updating, modification), retrieval, use, transfer (disclosure, access) to the extent and in the manner necessary for the purposes stated above, anonymization, blocking, erasure, and destruction of personal data.


4. METHOD OF PROCESSING

Personal data is processed by any lawful means, including with the use of automation tools and without such tools, within the Operator's personal data information systems.


5. DISCLOSURE TO PROCESSORS

I have been informed of, and agree to, the fact that in order to technically support the receipt and storage of requests, the Operator discloses my personal data to the following parties acting as processors of personal data:
— Yandex services — for the operation of the Site and/or analysis of its traffic;
— the Tilda platform and CRM — for receiving and processing requests.

These parties process personal data solely for the purposes defined in this Consent and the Policy, and are obligated to ensure the confidentiality and security of such data. The Operator does not carry out any cross-border transfer of personal data.


6. VALIDITY PERIOD OF THIS CONSENT AND WITHDRAWAL PROCEDURE

This consent remains valid until the purposes of processing are achieved or until it is withdrawn, whichever occurs first.

I am entitled to withdraw this consent at any time by sending a request to the Operator's e-mail address hello@drhotaki.com with the subject line "Withdrawal of consent to the processing of personal data". Upon withdrawal of consent, my personal data shall be destroyed, unless otherwise provided by the legislation of the Russian Federation.


7. RIGHTS OF THE DATA SUBJECT

The provisions of Federal Law No. 152-FZ "On Personal Data" have been explained to me and are understood, including my right to:

— receive information regarding the processing of my personal data;
— request the clarification, blocking, or destruction of my personal data if it is incomplete, outdated, inaccurate, unlawfully obtained, or is not necessary for the stated purpose of processing;
— withdraw my consent to the processing of personal data;
— protect my rights and legitimate interests, including recovery of damages and/or compensation for moral harm through judicial proceedings;
— appeal the actions or omissions of the Operator to the authorized body for the protection of the rights of personal data subjects (Roskomnadzor) or through judicial proceedings.

8. FINAL PROVISIONS

This consent is given by me freely, of my own will and in my own interest, and remains valid until the purposes of processing are achieved or until the moment of its withdrawal in the manner specified in Section 6.

By submitting the request form on the Site, I confirm that I have read and agree to the terms of this Consent and of the Operator's Policy, and I give my consent to the processing of my personal data on the terms set out herein.
